Introduction: This survey is to assess the 2013-2014 season in order to identify ways of improvement moving forward. Answers are requested in a manner consistent with constructive criticism, not a complaint form. If you choose to provide a written response please do so in a respective manner (ie. no profanity, name calling, etc.).
All assessments will be based on a scale of 1-5 with 1 being unsatisfactory, 3 being acceptable or neutral and 5 being completly satisfactory. For any scores below 3, please provide some response to indicate why you feel a catagory is below acceptable/neutral.
Only the League President will view submitted forms. (Online submissions go directly to the President and nobody else.) Once all forms are collected, responses will be summarized into an assessment report and distributed to all organizations. No names or teams will be specifically identified in the final assessment summary report. Please be as open and honest as possible as your responses are intended to improve the league.
